ENROLLMENT REQUIREMENTS FOR PRE-KINDERGARTEN STUDENTS

1. Original of the updated Vaccine Certificate- Form P-VAC 3 (See requirements at the bottom of the page)
2. Copy of the Vaccine Card.
3. Medical Certificate (form in the Application for Admission)
4. Original and copy of the Birth Certificate (the original is returned)
5. Original and copy of the Baptism Certificate (the original is returned)
6. Original and copy of Psychometric Exam (psychologist of your choice) (original is returned)
7. 1 2x2 portrait

ENROLLMENT REQUIREMENTS FOR KINDERGARTEN STUDENTS

The student must be 5 years old on or before December 31, 2021. The following requirements will be delivered with the Enrollment Application.

1. Pre-Kindergarten Transition Passport (Head Start). (If apply)
2. Original of the updated Vaccine Certificate - Form P-VAC 3 (See requirements at the bottom of the page)
3. Copy of the Vaccine Card.
4. Medical Certificate (form in the Application for Admission)
5. Original and copy of the Birth Certificate (the original is returned)
6. Original and copy of the Baptism Certificate (the original is returned)
7. Original and copy of Psychometric Exam (psychologist of your choice) (original is returned)
8. 1 2x2 portrait
9. APRESTO exam (free of cost) (you will be informed of the date after registering)
* In case the student deserves any other test, it will be indicated.

The student must be 6 years old on or before December 31, 2021. The following requirements will be submitted with the Application for
Enrollment.

{2.50} or more)
2. Letter of Conduct - in sealed envelope (General Conduct Index of 80 {2.50} or more)
3. Original of the updated Vaccine Certificate - Form P-VAC 3 (see requirements at the bottom of the page)
4. Copy of the Vaccine Card.
5. Medical Certificate (form in the Application for Admission)
6. Original and copy of the Birth Certificate (the original is returned).
7. Original and copy of the Baptism Certificate (the original is returned)
8. Original and copy of First Communion Certificate, if applicable. (original is returned)
